EPP MEMBER DNA

This form is only required if you are joining the Elite Pool Program and have already received, signed and sent back your buyer/seller agreement.

The following form is a DNA (Design Needs Analysis) that will assist us in building your production and allow us to customize everything to your business. Only what is on this DNA form is what is covered in your initial setup cost. Anything outside of this DNA form that you want setup may occur addition charges as the one time setup fee only covers a certain block of time.  

Please complete as thoroughly as possible and give as much detail as you can. Read the questions carefully so that the correct info is submitted to us. Incomplete or in-correct information will lead us to asking you to complete this DNA form again. 

There is 7 steps to this form, so please ensure you have the time to complete as it does not save progress.
Roughly half hour to 45 min.


There are 4 built in website design themes.  You will need to choose which one you want us to install. 
PLEASE DO NOT BASE YOUR DECISION ON COLORS, IMAGES,  TITLES,  LINK LABELS, CONTENT ETC, AS THOSE ARE ALL CUSTOMIZED TO YOUR BRAND AND OFFERING. WE ONLY WANT YOU TO LOOK AT THE STRUCTURE OF EACH THEME.  ANY ELEMENT CAN BE CHANGED BUT THE POSITIONING OF WHERE THOSE ELEMENTS ARE CANNOT BE CHANGED.  SO IT IS THE FRAMEWORK OF HOW THE DESIGN IS LAID OUT THAT YOU NEED TO EVALUATE AS TO WHICH YOU WILL SELECT.
